Mental Health Kokua - Case Management


Population Served
MHK's Case Management program is dedicated to adults with mental illness. “Case management/ services coordination programs provide goal-oriented and individualized supports focusing on improved self-sufficiency for the persons served through assessment, planning, linkage, advocacy, coordination, and monitoring activities. Successful service coordination results in community opportunities and increased independence for the persons served” (CARF).

Hours of Service
Monday:              8:00am - 5:00pm (after hours crisis, on-call available)
Tuesday:             8:00am - 5:00pm (after hours crisis, on-call available)
Wednesday:      8:00am - 5:00pm (after hours crisis, on-call available)
Thursday:          8:00am - 6:00pm (after hours crisis, on-call available)
Friday:                8:00am - 6:00pm (after hours crisis, on-call available)
Saturday:           Closed (crisis on-call available)
Sunday:              Closed (crisis on-call available)
 
Frequency of Services
Services are provided according to acuity; e.g., daily, weekly, semi-monthly, monthly.

MHK’s Case Management program is funded by:
Hawaii State Dept. of Health-Adult Mental Health Division
Ohana Community Care Services
Kaiser Permanente
 
Fees
MHK provides the services with no fees, or co-pays.
 
Services Provides
Services based on the strengths, needs, abilities and preferences of persons served.
Models of case management based on national standards and evidence-based care.
Levels of care based on monthly acuity assessments.
A comprehensive person-centered plan for each person served, including co-occurring conditions.
Linkages with community services and mobilizing involvement from the persons served support network.
Ongoing engagement of persons served in treatment through relapse and recovery.
 
Eligibility for IOP
18 years old and older
Diagnosed with a major mental illness
Meet the HUD definition of chronic homelessness
